I, Mohammed Irfan Qureshi, son of Mr. Abdul Rashid Qureshi, aged about 34 years, by occupation pharmacist, residing at 27, Ahmedabad Palace Road, Koh-e-Fiza, Bhopal, Madhya Pradesh 462001, holding Aadhaar no. XXXX XXXX 3178, do hereby solemnly affirm and declare as under:

  1. That I am a citizen of India and the deponent of this affidavit, and I am fully aware of the facts stated below.
  2. That my name is recorded as "Mohd. Irfan" in my Class X certificate issued by the Board of Secondary Education, Madhya Pradesh, in 2008, Roll No. 2143876.
  3. That my name is recorded as "Mohammed Irfan Qureshi" in my Aadhaar card, PAN card and B.Pharm degree.
  4. That both names refer to one and the same person, that is me, and that I shall be known as Mohammed Irfan Qureshi for all purposes in future.
  5. That this affidavit is being made for the purpose of submitting it to the Regional Passport Office, Bhopal with my passport application, and for no other purpose.
  6. That the statements made above are true and correct, and nothing material has been concealed.
