I, Teresa Marie Lindqvist, being first duly sworn, state as follows:

1. I am over 18 years of age and competent to make this affidavit. I have personal knowledge of the facts stated below.
2. I live at 3127 Bryant Avenue South, Minneapolis, MN 55408. My date of birth is April 22, 1988.
3. From January 15, 2020 to the present, I have lived continuously at the address above with my spouse, Anders Lindqvist, and our daughter, Elsa Lindqvist, born June 3, 2017.
4. Our home is owned by my mother, Carol Bergstrom, and for that reason no lease or utility bill at this address is in my name.
5. Elsa lives with me at this address full time and will attend a Minneapolis Public Schools elementary school for the 2026–27 school year.
6. A true and correct copy of my Minnesota driver's license showing this address is attached as Exhibit A.

I declare under penalty of perjury under the laws of the State of Minnesota that the foregoing is true and correct.
